The patient is a 16 year old who wanted a rhinoplasty to fix the “bump” on her nose. She also thought her nose was large for her face. She said she wouldn’t mind looking different if it would help improve her nose and the overall facial appearance.

Path to a New Style

That the patient had a vertically short nose was discussed and  the emphasis for her rhinoplasty would be towards retaining vertical nasal length while bringing harmony to the entire nose. The tip was to be rotated up slightly to lessen the impression that the tip was pointing downwards. The tip was also rotated  upwards so the nostrils could be in better position (more visible in the front view). Moving the tip upwards would also open up the area of the upper lip (side view) making the upper lip appear longer and less “tight”. The tip would be rotated without any shortening of the nose. Because the patient had a relatively wide face the nose would only be narrowed slightly to avoid disparity between the nasal and facial width.

Rhinoplasty Results

The patient was very happy with the results of her surgery and she felt her nose went with her face.

We can’t ignore the fact that when we change the nose, we change its relations with the face and that makes the face look different. So rhinoplasty is planned to not only to improve the nose but also the face. Making the nose smaller lets one see the other beautiful parts of the face. And changing the dimensions of the nose can emphasize or deemphasize facial dimensions.  Shortening/lengthening or widening/narrowing  the nose can also create spaces and so bring proportion to the face. For example, turning the nose up slightly in this patient makes the upper lip look longer and one can better see the appearance of the nostrils and the shape of the philtral columns (dimple in the upper lip) and this has brought beauty to the area under the nose.
